Your protected health information includes your symptoms, test results, diagnoses, treatment, health information from other providers, and billing and payment information relating to these services. We will not use or disclose your health information to others without your authorization, except as described in this Notice, or as required by law.
Our office asks for a 48 hour notice to reschedule or cancel appointments. Patients who do not show up to a scheduled appointment, without a cancellation phone call 48 hours prior to their appointment, will be subjected to a $95 No Show Fee.
